Join Our Community of Monthly Donors Empowering Patients To Live Healthy Lives

As an Evergreen Health Foundation GEM (Give Every Month) Society member you are part of a community that is deeply committed to sustaining the mission of Evergreen Health. Starting at $10/month ($.33/day) or a level of your choosing, your donation will make a positive impact on the lives of patients. Commitment levels include:

Ruby Level
Monthly contribution of $10 ($.33/day):
Can contribute to the Patient Enrichment Fund to support patient in a variety of ways including a co-pay or bus pass.

Topaz Level
Monthly contribution of $42 ($1.40/day):
Can purchase a tank of gas for our Mobile Unit to help serve the eight counties of Western New York in reducing incidences of overdose deaths and hepatitis C and HIV infections.

Emerald Level
Monthly contribution of $84 ($2.80/day):
Can provide a glucometer and test strips for two patients living with diabetes.

Sapphire Level
Monthly contribution of $208 ($6.90/day):
Can provide a home blood pressure monitor for two patients with hypertension.

Diamond Level
Monthly contribution of $417 ($13.90/day):
Can provide household goods and furniture to furnish an apartment for one patient.

As a member of the Evergreen Health Foundation GEM Society, you can expect:

Is my gift to the Evergreen Health Foundation GEM Society safe and confidential?

Absolutely! All of our donation processing is securely processed and PCI compliant.

Can I change my monthly gift amount?

Yes, you can change or suspend your gift amount at any time. Simply call 716-847-2441, email foundation@evergreenhs.org or write to us at Evergreen Health Foundation, PO Box 1106, Buffalo, NY 14201. We will be happy to promptly make any changes you request.

Can my monthly commitment go towards other program areas at Evergreen Health?

Of course, please email us at foundation@evergreenhs.org and let us know what program area you are interested in donating to.

How will I keep records of my monthly gifts?

You will receive a monthly email confirmation receipt of your gift. In addition, each January you’ll receive a letter from us detailing the dates and amounts of all your contributions throughout the preceding year.

Will I still get news about the Evergreen Health Foundation GEM Society?

Yes! You will receive regular updates on the progress being made at Evergreen Health.
