We know that following a health regimen is hard if you’re worried about where you’re going to stay at night. We provide housing assistance programs so you feel safe, secure and ready to focus on your health goals.
Our housing case workers can help you find funding or housing placement in Buffalo and Jamestown, and throughout all eight counties of Western New York.

The Evergreen Lofts provide long-term supportive housing opportunities for people who are homeless or are at risk of becoming homeless, and those with chronic illnesses.
The Evergreen Lofts is located in the Pratt-Willard neighborhood at 392 Genesee Street in Buffalo. The historic building has been completely renovated and has 56 units. Applicants must have income at or below 60 percent of the area median income, which is around $35,000 for a two-person household.
