Evergreen Health’s Primary and Specialty Care offers pediatric and adult medical and preventive services including annual exams, immunizations, and sick visits. Our team works with you toward identifying and achieving your individual health and wellness goals in an affirming environment.

As a certified Patient-Centered Medical Home, Evergreen has a model of care in which patient treatment is coordinated through your primary care provider. This includes expanded office hours and 24/7 access to a medical provider by phone. We ask you to contact our office before going to an emergency room so we can best coordinate your care. For minor, non-life-threatening illness or concerns when the office is closed, please call our answering service at 716.847.2441.

Evergreen Health accepts a wide range of insurance plans and does not turn patients away due to their inability to pay. For uninsured or underinsured patients, we offer a sliding fee discount program to provide care to patients at a cost they can afford.
