Evergreen Health’s Primary Care team offers medical and preventative services in our affirming and non-judgmental health center. We provide continuous care and work with you to achieve your individual health goals. This includes preventative services, like annual exams and immunizations, and sick visits. Our team will give you the tools you need to live a healthy life, but we won’t force you to do or be anything in order to come to Evergreen Health. We welcome everyone, and we don’t judge.

As a certified Patient-Centered Medical Home, Evergreen has more ways for you to get connected to the care you need when you need it. This includes having expanded hours on Tuesdays and providing 24/7 access to medical information by phone. For minor, non-life threatening illness or concerns when the office is closed, please call our answering service at 716-847-2441 to be connected to a provider instead of going to the emergency room or urgent care. The Patient-Centered Medical Home approach means care is personalized to you so you can be successful in achieving your health goals.
Our Primary Care team will coordinate with our Specialty Care teams to ensure you get the care that you need. Our teams specialize in HIV, hepatitis C and transgender care.
Founded as AIDS Community Services in 1983, Evergreen historically addressed the crisis generated by HIV and AIDS. Today, HIV is a manageable chronic health condition and is often treated with a simple daily prescription pill regimen. We offer comprehensive HIV services to help you manage your HIV and live the life you want, including: medical care and treatment, pharmacy services, insurance enrollment, individual and group counseling, case managers and care coordination, and more.
We are committed to providing HIV education and prevention services to our community. We offer HIV testing and treatment and are the leading WNY provider for PrEP, a drug taken once daily for HIV prevention.
Evergreen is a safe, affirming health center that provides sensitive care. We understand the health concerns of the lesbian, gay, bisexual and transgender community, and specialize in transgender health services, including: medical care, counseling, educational/social groups and more. We are dedicated to helping our patients feel safe, valued and welcomed.
While hepatitis C can be cured, treatment can be hard to find or stick with. We offer hepatitis C testing, treatment, pharmacy services, counseling and more. We can help you get into and succeed in treatment to cure your hepatitis C.
