Patient-Centered Medical Home

As a certified Patient-Centered Medical Home, Evergreen has a model of care in which patient treatment is coordinated through your primary care provider. This includes expanded office hours and 24/7 access to a medical provider by phone. We ask you to contact our office before going to an emergency room so we can best coordinate your care. For minor, non-life-threatening illness or concerns when the office is closed, please call our answering service at 716.847.2441.

Evergreen Health’s Primary Care Department was recognized for our outstanding patient outcomes in the fields of diabetes, high blood pressure, and high cholesterol.

Unconditional Medical Care

Evergreen Health accepts a wide range of insurance plans and does not turn patients away due to their inability to pay. For uninsured or underinsured patients, we offer a sliding fee discount program to provide care to patients at a cost they can afford. Our Financial Counseling Center is here to help you with all the financial components of your healthcare.
