We’re closed for Independence Day

All Evergreen Health locations and Community Access Services will be closed on Friday, July 4 in observance of Independence Day. We will reopen for regular business hours on Monday, July 7.

Meet our new Primary Care provider, Sarah Thomas, PA

Sarah Thomas, PA, is now providing Adult Primary Care on the 2nd floor at our 206 S. Elmwood location, in addition to Adult Acute Care on the Mezzanine Level (take the blue elevator by the Pharmacy up to the second level). She is accepting new patients and is looking forward to meeting you!

Sarah is originally from Indianapolis, and completed PA school at DeSales University in eastern Pennsylvania before moving to Buffalo. She loves dogs (especially pitties), reading, and traveling!

Visit Josh Matecki, PA at 206 S. Elmwood’s Mezzanine Level

Josh Matecki has relocated within the building at 206 S. Elmwood Ave. He will now be seeing patients on our Mezzanine Level (take the blue elevator by the Pharmacy up to the second level).

To ensure a smooth and timely visit, please plan to head to the Mezzanine Level for any future scheduled appointments. Our staff will be happy to assist if you need any help finding his new office.
