At Evergreen, we don’t just hope that our patients have a great experience when they come to us: our team includes a patient experience manager who’s dedicated to improving your experience and making sure that your feedback is heard and put into action.
Every interaction you have with us is part of your “patient experience” — from your phone calls and Patient Portal messages, to your appointments, to how you use our buildings and facilities.
We believe the voices of our patients belong at every decision-making table. Our patient experience manager is here to make sure your feedback is heard and transformed into meaningful change.
What happens when you share feedback with us?

Your feedback lets us know what we’re doing well and what we can do to grow. Our patient experience manager sees all the feedback our patients give, whether it’s through:
- our anonymous Patient Feedback Survey
- our patient feedback inbox: patientexperience@evergreenhs.org
- a phone call to our patient experience manager (716.847.2441, wait for the operator and ask to be transferred)
- an online review
While you’re welcome to leave feedback anonymously, if you include your contact info, our patient experience manager will reach out to you personally to talk through what happened and what solution you’d like to see.
