Report to Community 2018: Connect to Care

The world of healthcare is ever-changing, but at Evergreen Health, our focus is always our patients. We continue making strides to reach more people in the community who need our services and engage them in care. That’s why this year’s report theme is “connect to care.”

In 2018 alone, we opened two new service locations and developed numerous partnerships to improve health outcomes on a larger scale. We also saw a 31 percent increase in patients in 2018 and are proud to say that 31 percent of our patients are enrolled in two or more programs at Evergreen.

It is our goal that every Western New Yorker has access to affordable healthcare in an inclusive, compassionate, respectful and judgement-free environment, and we’ll continue working tirelessly to achieve it.
