All Evergreen Health locations and Community Access Services will be closed on Monday, September 1st in observance of Labor Day. We will reopen for regular business hours on Tuesday, September 2.

If you have a medical emergency during this time, please call 911. If you have a mental or behavioral health emergency during this time, please call 988.

Get your flu shot for a healthy fall

Flu season is coming! The best way to stay healthy this fall is to get your annual flu shot. Getting your flu vaccine is quick and safe, and it helps protect you and your loved ones from serious illness, especially during the colder months.

Ask your provider about getting your flu shot at your next visit, or call us at 716.847.2441 (press option 5) to schedule. You can also drop in at one of our Pharmacy locations (206 S. Elmwood Ave. or 800 Hertel Ave.) to get your flu shot!

Patient Portal scheduling is available for annual visits

Scheduling your annual check-up or a routine follow up appointment with your Primary Care provider is easy! Just log into your Patient Portal and navigate to the “Appointments” page to schedule an appointment.

September is National Suicide Prevention Awareness Month

If you’re struggling with your mental health, you’re not alone and help is just a moment away. Keep these numbers on hand in case you or a loved one needs support during a crisis.

  • 988 is an easy-to-remember number that you can call or text in a mental health crisis to get connected with a trained and caring counselor. You can use 988 for thoughts of suicide, substance use crisis, or other times of emotional distress. 988 is free and available to everyone. Call or text 988.
  • The Trevor Project offers a 24/7 crisis line especially for LGBTQ+ youth, who face an especially high risk of suicide. Text “START” to 678-678 or call 866-488-7386.
  • The Crisis Text Line offers 24/7 support with volunteer crisis counselors via text. Text “HOME” to 741-741.
