Evergreen Health offers nutrition education classes and assistance for people who are living with chronic health conditions who want to learn how the foods that they eat affect their overall health. With this program, learn how to give your body the best nourishment for your unique health needs and goals.
Good nutrition can help delay the progression of many chronic health conditions. When you’re well-nourished, it can be easier for your body to manage your symptoms and allow your medications to work better.
While working with our friendly nutrition team, it may be determined that you also qualify for additional assistance, including monthly food vouchers to assist in purchasing approved food items, or volunteer shoppers to assist if you have difficulty food shopping.
Our Nutrition program has limited availability and is offered to patients through referrals. Please call your Evergreen provider to learn if you meet medical and/or financial eligibility requirements.
As a member of the Food Bank of WNY, our food pantry is open to eligible patients of any service at Evergreen or residents of 14201. Patients must call ahead to schedule a pantry service. The pantry is open 9 a.m. – 1 p.m. and pick-up is able to be scheduled Monday through Friday to align with other patient appointments when possible. Our pantry waiting room remains closed and patients can pick up their curbside pantry at the scheduled time at the designated side door on Chippewa. Call us at 716.847.2441 with any questions or to set up a pantry pick-up.
