Nutrition at Evergreen

Evergreen Health offers nutrition education classes and assistance for people who are living with chronic health conditions who want to learn how the foods that they eat affect their overall health. With this program, learn how to give your body the best nourishment for your unique health needs and goals.

Why is nutrition important?

Good nutrition can help delay the progression of many chronic health conditions. When you’re well-nourished, it can be easier for your body to manage your symptoms and allow your medications to work better.

What you’ll learn

  • food safety
  • cooking with items from a food pantry
  • budgeting for food shopping
  • managing the symptoms of chronic health conditions
  • changing food habits and behaviors
  • basic health and nutrition
  • how to make healthy food choices

While working with our friendly nutrition team, it may be determined that you also qualify for additional assistance, including monthly food vouchers to assist in purchasing approved food items, or volunteer shoppers to assist if you have difficulty food shopping.

Requirements to join

  • attend one group or individual appointment every month
  • meet with a dietitian once or twice a year for an assessment
  • participate in the program for 12 to 18 months

Our Nutrition program has limited availability and is offered to patients through referrals. Please call your Evergreen provider to learn if you meet medical and/or financial eligibility requirements.

Food Pantry

As a member of the Food Bank of WNY, Evergreen offers a food pantry to eligible patients and community members in the 14201 zip code. A three-day supply of food is provided for individuals and their family members. The Food Pantry also offers a Mobile Food Pantry Express Truck once per month with fresh produce and perishable items.

*The Pantry is open every Monday from 2:30-4:15 p.m., Tuesday from 2:30-5:45 p.m., and Wednesday and Friday from 9:30-11:15 a.m.
